You'll save time on your first visit if you take a few minutes now to gather the items you'll need for that first appointment. If it's convenient, you can even print and complete the necessary forms and bring them with you to our office. In any case, please plan to arrive 15 before your scheduled appointment to complete your registration. Thanks!

• Written Acknowledgement Form
• Disclosure to Family & Friends Form
• Privacy Policy Form
• Insurance card
• Referral forms, if necessary
• Copies of pertinent diagnostic tests and medical records
• A list of all current medications
• A 'New OB' form, if applicable
